Vital Records

To obtain copies by mail please provide the request form and the following:

Valid Photo I.D. (person making request)
$20 in a check or money order Payable to: New London City Clerk
Request in writing or fill out birth request form (either one is acceptable)
Self-addressed Stamped Envelope (This will be used to mail back your record inside)

PROVIDE TWO (2) documents FROM LIST BELOW with your request:

Social Security card; Social security card supplemented with either an employment I.D. card, a paycheck stub or a W-2 form. Providing both of these documents fully satisfies the I.D. requirements
Motor vehicle registration
Utility bill showing name and current address
Checking account deposit slip or bank statement stating name and current address
Voter registration card
Valid government issued trade or professional license
Valid government issued firearm permit
Probation documents issued by court or other government agency, pursuant to a criminal conviction
Letter from a government agency verifying identity. The letter shall be dated within six months prior to the date of the request
Military discharge papers
Current school or college photographic identification
Government issued photo I.D. that ahs expired within 12 months prior to date of the request
